BDHeaRT Mobile medical Team – Decentralization of activities

This is to share the information on the decision of the decentralization of the medical mobile team that happened from 1st March 2005.
Following the discussion during the BDHeaRT coordination meeting on 19th February and the same was decided and implemented.
In regard to that, the medical teams operating and planning to provide services in the Batticaloa district are asked to contact Dr. Sathurmugham, DPDHS and get accredited before initiating their activity.
The DPDHS will direct the medical team to the MOOH in the priority division.

The potential roles of the Medical Team

The medical teams meet the MOOH of their area of intervention and plan out for the schedule. The suggestion is that those camps that are far from the health institutions might need a doorstep medical attention at this non emergency phase.

Depending on the need in the particular area, the medical team also link up with DMO of the District / Base Hospital through the MOOH to provide medical care at the hospital.

The medical team collects the necessary required details regarding the consultations provided and submits it to the MOOH for the documentation.

When possible, the medical team brings their drugs for prescription. The drug lists are validated with the respective MOOH for the expiry, usage, etc.

If not available, the drugs could be obtained from the MOOH of the respective division.

BATTICALOA DISASTER HEALTH RESPONSE TEAM - Activities Update -18 February 2005

Introduction

The Batticaloa Disaster Health Response Team (BDHeaRT) is coordinated and maintained by the offices of the Deputy Provincial Director of Health Services (DPDHS), Batticaloa Teaching Hospital, Terre des homes (Lausanne), Handicap International and other concerned organisations/individuals. The immediate objective of BDHeaRT is to provide urgent medical assistance to people in the Batticaloa District of Eastern Sri Lanka affected by the tsunami of 26th December and its aftermath.

BDHeaRT’s current headquarters are at the DPDHS office (Tel: +94-65-2222931) in Batticaloa Town. The primary contact person for BDHeaRT is Dr. M. Sathurmugam (+94-776915674).

MOBILE MEDICAL TEAMS

During the first month, BDHeaRT was operating 10 teams daily. At present aprox 4 -5 mobile medical teams (approx) are dispatched every day. The visits are being held at the temporary camps/shelters/ resettlement areas in the Batticaloa district.
· The feedback obtained during the past 2 weeks shows that the situation is no longer an emergency and the disease pattern is near normal with very less direct Tsunami affected people. Considering this, we reduced the frequency of the visits from twice a week to once a week – 10 days for the nearer camps.
· Now that the emergency phase is over, participating teams have recognized the necessity to give clear messages to the communities so that they can receive medical care in sustainable existing health structures. Handicap International will develop such messages and will start programmes in coordination with DPDHS and field health structures.
· Awareness messages on hygiene and prevention of diseases and preparedness for the emergency situation should be developed.
· The teams commonly deal with psychosomatic cases which are referred to the psychosocial committee.
· The outcome of the focused discussion organized with the different stakeholders reveals that there is a need to look into a sustainable way of delivering services.
· Coordination for the reconstruction for the health Institutions damaged should be developed. Handicap International will provide specific inputs / documentation on Barrier free Environment.

BDHeaRT’s DATA COLLECTION & SURVIELLANCE MONITORING
– A note

Responding to the TSUNAMI that struck on 26th December 04, the medical mobile team coordinated by BDHeaRT (Pls refer the document on BDHeaRT) provided acute medical care for the internally displaced people at the camps in the Batticaloa District.

Everyday 8 to 10 teams (that included a coordinator; 2-3 Doctors; 3- 4 nurses and students) were sent to provide the services on the second day itself.

As BDHeaRT being the first team to respond immediately at the camp level, the coordinators looked in to the comprehensive issues like food, clothes, water, sanitation, etc. the details furnished into the database. (Phase 1 mentioned in the database),
The information obtained from the camps is compiled that gave us an idea of what the immediate needs of camps. The data was shared with the appropriate organization which acted as baseline information.
The mobile team also attempted to provide specific information like unaccompanied children details and referral for respiratory conditions, etc.

From 3rd January, after receiving expert inputs from Regional Epidemiologist, Batticaloa and the Epidemiology Unit, (Colombo) we made amendments twice (Phase 2 & 3) on the contents of the forms to understand the disease pattern and the surveillance in the district.
During the later stage of the phase, the team eliminated exploring the other needs (food, clothes) which were sufficiently catered by the other organization.

USAGE:
To understand the incidence of particular kind of disease that existed during the visit.
To relate on the types of diseases incidence for the surveillance monitoring and to make priority for the next medical team visit.

PROCESS:
The medical mobile team started with addressing the acute medical conditions including the wound dressing, acute infection and presently expanded their services to wide spectrum of disease conditions including patient with chronic diseases.

Till date, the team is able to control any epidemic breaking out in camps and the condition inside the camp is also maintained by the other organisations.

The subjective diagnosis and the Medical Team’s approach from the different context always existed during the camp visits. BDHeaRT attempted to regularize on the drug list which is commonly used in this part of the world and disseminated the Srilankan Government’s chemotherapy recommendations for specific interventions (malaria) were provided.

DISSEMINATION:
The database is updated daily and the updated information is shared with the authorities regularly. Attempt is also made to include in the web with all the other administration difficulties.

However, two weeks on from the tsunami disaster, we would like to acknowledge the roles played by a number of persons and agencies. The following list is in no particular order and is not exhaustive. We regret if we have failed to record or have mis-identified anyone who helped out – but there was little opportunity to take down details in the rush of the initial emergency response.

BDHeaRT Operations Update 08 January 2005

Since the emergency medical response in Batticaloa began to be coordinated on Tuesday 28th December 2004, over 176 mobile medical clinics have been held in and around the camps housing people displaced by the tsunami in the Batticaloa District. The number of treatment consultations as of 07 Jan 2005 was 19,307. The teams conducting these clinics have been coordinated by the Batticaloa District Health Response Team and have been comprised of doctors, nurses, public health midwives, public health inspectors, nursing students and volunteers from the DPDHS, Batticaloa General Hospital, districts of Colombo, Kandy, Mannar, Badulla, Negombo, Peradeniya, Matale & Polonnaruwa, Tamils Rehabilitation Organisation, Medicins du Monde (Greece), Handicap International, Terre des Hommes, Rupavahini Corporation, Rotary International, Malaysia, USA, Canada and elsewhere. Additional support has been provided by pharmacists and volunteers from Batticaloa General Hospital, Volunteer Services Overseas, Sri Lanka Red Cross and elsewhere.

Since last week, the population in the camps and shelters for displaced persons has dropped significantly. As of yesterday (07 Jan 2005) it was reported that there were approximately 17,500 families resident at 68 locations within the Batticaloa district. The Eastern University, Batticaloa estimated that around 9,500 persons were living with friends and relatives and that over 36,000 people had returned to their villages. Further information is available at http://www.eusl.info/ and from the office of the Government Agent Batticaloa. Many camps are being closed and residents are being moved to other formal and informal shelters. These shifts within the population directly affected by the tsunami have clear ramifications for the work of BDHeaRT and the other medical service providers in the district.

To date, the work of BDHeaRT has been to provide emergency medical treatment, to support basic preventative measures and to conduct surveillance for communicable diseases that might pose a threat of an epidemic within the displaced population. The coordination of medical teams active in the district was intended to ensure the following: a) that there was consistent and equitable health coverage for the affected population; b) that medicines used are consistent with standard practice in Sri Lanka; and c) that there was systematic reporting of diseases that posed a threat of epidemic. Data gathered has been forwarded to the epidemiological unit at the Ministry of Health, Sri Lanka, which also provided advice to BDHeaRT on disease surveillance. Information related to water and sanitation needs of temporary camps was also forwarded to key relief agencies dealing with these issues within the district. Additional information (such as that pertaining to unaccompanied or separated children) was also shared with the appropriate agencies.

The support provided by local and visiting health personnel, NGO staff and volunteers was remarkable from the outset. Although the first week of operations were marked by a near-shortage of human and material resources, by the second week of work, the situation had changed to one of surplus. In terms of medical personnel and transport, the influx of external service providers actually necessitated the reduction of locally-sourced medical teams in order to avoid the over-supply of curative medical services to camps. The rapid turn-over amongst the majority of visiting teams (average duration of 1 to 3 days), lack of appropriate language skills within these teams and shortages of support staff (ie. dispensers and nurses) meant that a large proportion of BDHeaRT resources were dedicated to coordination of these external teams. A smaller number of these visiting teams have been prepared to remain in the Batticaloa district for between two and eight weeks, and it has been possible to easily integrate them into a longer-term plan for service provision to affected areas. There have also been teams operating within the district that have failed entirely to coordinate with BDHeaRT/DPDHS office, despite a directive requiring this by the Ministry of Health. Consequently, there is no available information on the activities of these teams, the incidence of disease/injury encountered by them or the treatment they have provided.

Over the past few days, there has been an attempt to start shifting the emphasis of medical service provision away from mobile clinics coordinated through BDHeaRT and back to the main structures for health service delivery in the Batticaloa District. This shift is imperative in order to ensure sustainable and sufficient services in an area where existing structures had already been weakened by two decades of armed conflict prior to the tsunami. The DPDHS Batticaloa has made an appraisal of the damage caused by the tsunami to the pre-existing health service institutions in the district and this information is available at his office. There is a need, at present, to make a formal appraisal of the capacity of the main health services to meet the needs for maintaining good public health through active disease surveillance, preventative interventions and treatment provision. At present, it seems that a lack of medical personnel, public health knowledge, transport facilities, and infrastructure for rapid information gathering, sharing and analysis may all present substantial impediments to ensuring the good health of the affected population. The shifting of displaced and affected persons out of camps into the ‘general population’ actually increases, in many ways, the public health challenges. It is crucial that the invisibility of displaced persons within the general population does not lead to a perception that the health needs of such a population have necessarily reduced or disappeared.

Batticaloa Disaster Health Response Team (BDHeaRT) INFO

The Batticaloa Disaster Health Response Team (BDHeaRT) is coordinated and maintained by the offices of the Deputy Provincial Director of Health Services (DPDHS), Batticaloa Teaching Hospital, Terre des homes (Lausanne), Handicap International and other concerned organisations and individuals. The immediate objective of BDHeaRT is to provide urgent medical assistance to people in the Batticaloa District of Eastern Sri Lanka affected by the tsunami of 26th December and its aftermath.

BDHeaRT’s current headquarters are at the DPDHS office (Tel: +94-65-2222931) in Batticaloa Town. Information related to BDHeaRT is also available at http://www.bdheart.blogspot.com/. The current contact person for BDHeart is Dr. M. Ganesan (+94-776063898).

MEDICAL TEAMS
At present, approximately 10 medical teams are operating out of this office daily, holding clinics in the temporary camps/shelters set up to house persons displaced by the tsunami and subsequent bad weather. Each team is made up of 1 medical team coordinator, 2 biomedical doctors and 2 or 3 nurses. There are both preventative and curative components to the work of the medical teams. The preventative aspect is addressed by the coordinator and a doctor (Medical Officer for Health). They would focus on creating groups within camps to carry out the following activities: improve conditions of sanitation and hygiene; keep demographic records on the resident population; manage food stores and distribution. The coordinator will also advise residents of the camps on issues such as toilet cleaning, water sanitation and food hygiene. The team monitors the supply of water, the adequacy of toilet facilities, and also helps chlorinate the water source and disinfect toilets. Tennis balls or other simple play equipment is provided to children, as a very basic mental health promotion activity.

The teams currently visit camps on a rota that aims to provide medical services twice a week in each of the approx. 100 camp/shelter locations in the Batticaloa District. There is still a need to coordinate with other providers of medical services to ensure adequate, consistent and equitable coverage of affected populations. All medical service providers working in Batticaloa who are not yet coordinated with BDHeaRT are requested to make contact at the above address/number
- so that none of us waste scarce resources and time in duplicating or interfering with each other's work. BDHeaRT itself was formed through the consolidation of a number of separate efforts that developed during the initial days following the disaster.

The resources available at BDHeaRT to carry out the above activities are only barely sufficient at present. There are difficulties in obtaining adequate numbers of doctors and 4WD vehicles to staff the teams on a daily basis – the level of available resources fluctuate on a daily basis. At present attempts are being made to mobilise resources within the Batticaloa district (especially in the case of doctors). The availability of required drugs is also inconsistent. There has not been an adequate mobilisation of primary health care workers (Public Health Inspectors and Midwives) available in the district to augment and follow-up the preventative work of the medical clinics. Assistance/collaboration to address these and other shortcomings would be much appreciated.
